I went to a Chinese ER with chest pain. I was out in 3 hours with a $85 bill.

My friend from Texas felt chest pain after a spicy Chongqing hot pot dinner. He was 32, healthy, but panicking.

His Chinese colleague took him to Fuzhou First General Hospital. Emergency room. 8 PM on a Tuesday.

The process:

Step 1: Registration
Walked in. Showed passport. Paid ¥15 ($2.10) registration fee. Got a number.

Step 2: Triage
A nurse checked his blood pressure and heart rate immediately. Chest pain = priority. He skipped the queue.

Step 3: Doctor consultation
5 minutes later, a doctor saw him. Spoke basic English. Ordered tests:
• Blood work (CBC, cardiac enzymes)
• EKG
• Chest X-ray

Step 4: Testing
Blood drawn. EKG done. X-ray taken. All within 20 minutes.

Step 5: Results
Digital results appeared on the hospital's WeChat mini-program in real-time. Doctor reviewed them in 30 minutes.

Verdict: Gastric reflux from the hot pot. Not cardiac.

Step 6: Discharge
Prescribed antacids. Paid at the self-service kiosk. Left.

The bill:

ServiceCost (RMB)
ER registration¥15
Doctor consultation¥25
Blood work (full panel)¥180
EKG¥35
Chest X-ray¥80
Medication (antacids)¥30
Total¥365 ($51)

Wait — the total was actually ¥613 ($85) at some hospitals for a similar workup. Prices vary by city and hospital tier.

In Texas, the same ER visit would have cost $2,000-$5,000 even with insurance.

How Chinese hospitals are so fast:

China's public hospitals use a tiered system. Non-critical cases wait. Critical cases (chest pain, severe injuries) go first.

The hospital was massive — 3,000+ beds. But it was built for throughput. Multiple testing stations. Digital results. No waiting for a radiologist to "read" your X-ray — the machine does preliminary analysis instantly.

The language barrier:

The doctor spoke basic English. The nurses didn't. My friend used Google Translate for the parts the doctor couldn't explain.

Tip: In major cities, top-tier hospitals (三甲医院) often have international departments with English-speaking staff. But they're 3-5x more expensive.

For foreigners:

Public hospital (standard):
• GP visit: ¥10-50
• Specialist: ¥50-200
• ER visit: ¥50-150
• Blood tests: ¥100-400
• X-ray: ¥50-150
• CT scan: ¥400-1,200
• MRI: ¥600-2,000

International clinic:
• GP visit: ¥800-2,000
• Specialist: ¥1,500-4,000
• ER: ¥1,000-3,000

Payment:
• Public hospitals: Pay upfront. Cash, Alipay, WeChat. No foreign insurance accepted directly.
• International clinics: Some accept foreign insurance. Direct billing possible.

Travel insurance:
Get it before you arrive. Most policies cover emergencies and reimburse you later. Keep every receipt.

My friend's reflection:
"I walked into an ER in China expecting chaos. I walked out 3 hours later with test results, a diagnosis, medication, and a bill smaller than my monthly gym membership. I still don't understand how that's possible."
